Reablement Support Worker

We are seeking compassionate and dedicated Reablement Support Workers to support our client on a temporary basis!

As a Reablement Support Worker, you will play a vital role in helping individuals regain independence and improve their quality of life through personalised care and support, as well as ensuring safe and timely transportation to and from appointments and community activities

Responsibilities

• Provide practical and emotional support to individuals in their own homes or community settings.
• Assist clients with daily living activities, encouraging independence and confidence.
• Safely transport clients to appointments, social activities, and community resources.
• Monitor and report any changes in client's health or needs, following safeguarding policies.
• Ensure all care practices adhere to relevant legislation and organisational standards.
• Maintain accurate and confidential records in line with agency compliance requirements.

Qualifications

• Previous experience as a Care Support Worker, Reablement Support Worker, or similar role is preferred.
• Valid driving license with a clean driving record.
• Experience driving clients safely, including those with mobility or other assistance needs.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a multidisciplinary team.
• Commitment to promoting equality, diversity, and inclusion in the workplace and care delivery.
• DBS check and other statutory clearances as applicable.
